Before We Meet

If you would like more information about my services or would like to set up an appointment, you may leave a message for me at the number to the right. I will return your call personally so we can talk about what you need.

After our initial conversation, if I think I can help, I will send you access to my secure portal. When you log in, you will find a few forms to complete. Some of these are required by law (such as the HIPAA form) and only need your signature. You will also find an “intake” form which will ask for specific and personal information so that I can better understand your history and current concerns. Complete these at your own pace then return them to me through the secure portal.

Once I receive your materials, I will review them carefully and establish a plan for our first meeting. I will contact you to set up an appointment.

Payments and Insurance

The cost of my services depends on the nature of the service you are seeking. During our first phone conversation, we can discuss the rate for the type of counseling or assessment you need.

Although I do not accept direct payment from insurance companies, most plans will provide an “out of network” benefit for non-participating providers. You would pay the fee at the time of our appointment, and I would provide you with documentation required by most insurance companies so that you can be reimbursed.

Location

My office is located at 6340 Sugarloaf Parkway, Suite 200 in Duluth GA. However, virtual sessions may be appropriate based on the type of service you are seeking.
